Creating / Editing a New Incident Form

The Create / Edit New Incident Form can be accessed one of two ways:

NEW => Incident Form

VIEW => Incident Forms

If you have the proper permissions, (Incident Form Create or Manager) you will also see the Add Incident Form button at the top left of the sub-navigation menu.

This is a single page module. Most of the actions performed in this module update/save instantly.

Incident Forms can be managed by more than one person, we recommend using the refresh button often to pull the latest data.

Because of the nature of emergencies, where multiple personnel may be working on a incident, incident forms are not locked to a specific person. Anyone with Manage or create permissions can edit or delete ANY existing Incident Form.

Creating a New Incident Form

To create a new New Incident Form, use the navigation menu and go to

NEW => Incident Form

OR

VIEW => Incident Forms then click on New Incident Form.

The Add Incident Form screen opens allowing you to add / update the incident.

As you enter / modify data, the form is saved and updated automatically.

Fill out the fields and as necessary.

Select or enter a new Type and/or Location. If you are entering in a new value, click the Tab key after entering in the new value.

If your agency chose to track arrival and departure times, those columns will be visible.

Click the check marks to document that person was on scene.

To track additional personnel not in your agency, use the Additional Persons section at the bottom of the form.

Editing an Existing Entry

To edit an existing entry, (Incident Form Create or Manager) from the Historical Form list, click the view button on the form you wish to edit.

This brings you to the Incident Form. Modify the form as needed. Changes are saved instantly.

Closing (Locking) an Incident

The employee who created the incident and persons with Incident Form Manage permission will see a Close Incident Form button at the top right of the form. Click that to close the Incident.

Those same people will also see a Re-Open Incident Form button when the Incident is closed. You can click Re-Open to re-open the incident at any time.

Deleting an Incident

If you have Incident Form Manage permission, you will see a Delete Incident Form button at the top right of the Incident Form.

Click Delete Incident Form to permanently delete the entry.

You will be prompted to type DELETE into a text field to confirm the deletion.

Deleting an entry is permanent, it is NOT recoverable.
